Parents fill in their child’s name, grade, and school.

They check the box to formally opt their child out of these activities.

They sign and date the form, which then must be kept in the child’s permanent school record.

The opt-out remains in effect until revoked in writing by the parent/guardian.

The form states that a child may not participate in:

Human sexuality education (including instruction on sexual topics, STDs, or gender identity).
Surveys or questionnaires (paper or digital).
Counseling, screenings, or referrals related to mental, emotional, or behavioral health.
